Choose and Book is a national service that enables
patients to choose the place, date and time they would like to
attend their first outpatient appointment. This enables GPs or
patients to book their appointments whilst at the GP
practice, or online or by phone when they get home.

How to Book
Appointments can be booked either via the Choose and Book
website, or by calling the appointment line on
0345 60 88 88 8. This service is available
between 7am - 10pm. If your patient is going to book their
appointment when they get home, they will need their reference
number and password.

'Advice and Guidance' is also available for all services at
Lewisham Healthcare NHS Trust through Choose and Book. This
